When you sign up for the CREW program, we may collect the following personal information:

•       Your diagnosis (PKU or TYR)

•       Information you provide during program activities (e.g., health goals, responses to surveys)

•       Notes from your telehealth consultation with an Exercise Physiologist (if applicable)

 

2.   How do we use your information?

We collect and use your information to:

•       Deliver the CREW Patient Support Program to you

•       Communicate with you about the program (e.g., reminders, resources, wellness tools)

•       Arrange your telehealth consultation with an Exercise Physiologist (if applicable).  

•       Provide your clinician or dietitian with a summary of your exercise consultation

•       Monitor and improve the CREW program

•       Comply with relevant laws and healthcare standards

We will only use your information for the purposes stated above or with your additional consent.

 

3.   Who do we share your information with?

We may share relevant information with:

•       Your primary healthcare team (e.g., dietitian or clinician) to support your care

•       Our contracted Exercise Physiologists delivering services as part of the program (if applicable)

•       Service providers assisting with program administration (e.g., mailing resources) — these providers are required to comply with strict privacy standards

We will never sell or share your personal information for marketing purposes.

 

4.   How do we store your information?

We take reasonable steps to protect your information from misuse, interference, loss, unauthorised access, or disclosure. Your information is stored securely within Australia, using password-protected systems and files.
